• CRA forms - $ 25

• Physiotherapy/Occuptational Therapy/ Massage prescriptions - $ 15

• Chart Transfers - First 50 pages - $ 25 Additonal pages - .30 each

• Simple sick note, daycare note or parents off work notes - $ 25

• Detailed Physician Letters/ Chart notes ( full page ) - $40

• Full Physical / Fitness Forms - $ 100

• Certificate of Fitness forms ( for school, daycare, camp or travel ) - $30

• MPI Drivers Medical Forms - $110

• Disabilty Tax Credit - $300

• Parking permit/handi transit application - $30

• Out of country & Quebec residents: Office visit - $75

• Out of country & Quebec residents: Complete Exam - $100

Injections: 

Twinrix ( Hep A&B ) + injection - $ 60 each dose

Injection only - $20 ( excluding Depo-Provera )
